99mTc-MIBI -SPECT and 18F-FDG-PET/CT in the staging of a non-Hodgkin lymphoma patient

Abstract. A case of a 59-year old man with through a cervical lymph node (LN) biopsy histologically confirmed follicular non-Hodgkin lymphoma (NHL) is presented. Computed tomography (CT), 99mTc-MIBI -planar scintigraphy and SPECT for excluding multidrug resistance (MDR) and a PET/CT scan for initial staging were performed. CT discovered pathologically enlarged right gastric and bilateral axillary LN. 99mTc-MIBI -SPECT showed pathologic uptake of the tumor-seeking radiopharmaceutical in both axillas, both inguinal regions and in the right-sided abdomen – probably paraduodenal region. The estimated uptake values and retention indices showed no suspicion of MDR. PET/CT revealed increased glucose metabolism in bilateral submandibular, jugular, axillary and inguinal LN as well as in paracaval and mesenterial LN. The possibilities of modern nuclear medicine methods in the assessment of lymphoma spread and the need of further studies about SPECT/CT as a more accessible alternative of the “gold standard” PET/CT are discussed.
